Head shot and assistant chef's name – you must submit a head shot of the lead chef along with the name and title of your assistant chef no later than Friday, January 16, 2012.

Recipe – Must be submitted by the lead chef no later than Friday, January 16, 2012. Accurate recipe ingredients and preparation instructions to be used as a criterion for judging and will also be published for use on the website and on other promotional materials.
